Prequalification questionnaires
Prequalification questionnaire support turns the forms that owners and general contractors send into organized answers drawn from facts you approve.
The service
What you send. What you get.
You send
- The questionnaire and any instructions or definitions the owner supplies
- Financial statements, the surety letter and insurance certificates
- Safety programs and statistics, licenses and project references
- Approved company facts: ownership, key personnel and experience
- Answers to earlier questionnaires you want reused, if any
- The name of the person who signs or notarizes
Start with a short description. We arrange document sharing after confirming the work.
You get
- Master facts sheetCompany facts drawn from documents you provide, each with its source and date.
- Answer mapEach question mapped to an approved fact and drafted in the form's own terms, for your team to confirm.
- Evidence index and gap listWhich document supports which answer, and which questions have no approved answer yet.
- Expiry trackerCertificates, licenses and statements listed with their dates, so renewals do not lapse before the next form.
Typical buyers: Subcontractors prequalifying with general contractors; General contractors prequalifying with owners and public agencies; Safety, finance and operations managers who assemble the same facts for every form.
What we check
Your bid requirements, organized.
The facts cover company and ownership data, financials, bonding, insurance, safety, experience, references and licenses. We build a master facts sheet, map it to each questionnaire and keep an evidence index and an expiry tracker. Your company answers, signs and submits. We confirm fit and a fixed quote after reviewing the questionnaire.
- Questions by form section, with the fact and document that answers each
- Company and ownership facts
- Financial statements, indexed by period
- Bonding capacity letter from your surety
- Insurance certificates and their expiry dates
- Safety programs and safety statistics, on the basis each form defines
- Licenses, references and key personnel
- Litigation and claims disclosures

Questions
About prequalification questionnaires.
How is this different from a qualifications package?
A qualifications package is a narrative response to a request for qualifications. A prequalification questionnaire is form-driven and data-heavy, and the same company facts are entered again and again. The AIA Contractor's Qualification Statement (A305), used with the Request for Contractor's Qualifications (A304), is one standard pairing of request and response forms. This page covers the questionnaire, and the qualifications page covers the narrative.
Can you fill in answers we have not approved?
No. Every answer comes from facts and documents your company provides and approves. We do not create or enlarge experience, safety data or financials. Gaps go on a list for your team to resolve, and your authorized person signs or notarizes the form where it asks.
Why keep a master facts sheet?
Contractors complete many questionnaires, and each asks for the same facts in different words. One approved source keeps answers consistent, and dated documents show when a certificate or license needs renewal. Public owners set their own requirements too: Florida DOT, for example, certifies bidders on larger contracts from a sworn application with CPA-prepared financial statements.
